Strategies to help primary care providers set realistic patient expectations about proton pump inhibitor (PPI) use and reduce over-reliance on PPIs are pertinent, particularly for patients who remain symptomatic during therapy.

This module will cover a multi-modal approach to the management of reflux in general practice, outlining how incorporation of on-demand medicines can support empiric PPI trial and address breakthrough symptoms that may occur in patients taking PPIs. Next steps for patients with refractory symptoms will also be summarised, ultimately helping general practitioners (GPs) to effectively respond when their patient asks, “why is my PPI not working?”

Learning outcomes


After completing this education module, participants will be able to:

  • Outline multimodal reflux treatment strategies incorporating on-demand medicines that can support empiric trial of PPI therapy for reflux symptoms
  • Outline reflux treatment strategies to manage breakthrough symptoms experienced by patients taking maintenance PPI doses
  • Identify patient variables that can contribute to failure of PPI therapy
  • Describe further investigations to be undertaken following failure of PPI therapy.
